Intensive Case Management

Intensive Case Management (ICM) provides mental health treatment services and supports that help individuals with severe mental illness achieve their recovery goals.

ICM services assist individuals in minimizing the negative effects of symptoms of mental health problems and addictive diseases that interfere with daily living, independent functioning and personal development.

Services help individuals develop strategies and support to avoid out-of- home placement, reduce the need for more intensive services and coordinate access to other needed community services and resources.

ICM Services can be provided in a clinic, at an individual’s home or in a community setting (i.e. homeless shelters, street environments); depending on the needs and preferences of the individual.

Who Intensive Case Management Services are for

Intensive Case Management Services are available for individuals living with severe and persistent mental illness that seriously interferes with their ability to live in the community, and one or more of the following:

Transitioning or discharged from a psychiatric inpatient facility in the past six months.

Admitted to a psychiatric inpatient facility or crisis stabilization unit for psychiatric stabilization and/or
treatment three or more times in past 12 months.

Continuously homeless for a year or more or four episodes of homelessness in the past three years.

Released from jail or prison in the past six months.

Seen in the emergency room for behavioral health needs three or more times in the past 12 months.

Transitioning or recently discharged from Assertive Community Treatment (ACT) services.
